Responsibilities

  • Conduct RN supervisory visits and initial/home safety assessments.
  • Create/update care plans; align with PCP/family input.
  • Train, coach, and evaluate CNAs/CMTs/companions on tasks, documentation, and safety.
  • Review visit notes, incident reports, and escalations; implement corrective actions.
  • Perform periodic case reviews and quality checks; maintain compliance with agency policies.
  • Participate in scheduling/coverage decisions for urgent needs (per diem/on-call).
